Punch insertion is only possible with three distinct 1.5 mm scalp layers, each of which has a different consistency. The deeper layers contain connective tissues and fatty tissue, making them softer. This may make them less resistant to being punched, which could result in damage if the hair grafts are not properly trimmed.
Extraction machines can perform cutting, rotating, oscillation and vibrations depending on the tissue stiffness. This causes minimal damage to the hair roots. The punch is kept close to the tip of the blade during harvesting in order to have better control.
This action can be performed automatically using a “Smart React” mechanism or manually, by pressing the pedal. This machine creates the oscillatory movement that harvests the hair grafts from deeper soft layers by sucking them out.

FUT or FUE? Which to choose and when?
- Hair transplantation is performed using FUE and FUT. The techniques have undergone many improvements to eliminate the disadvantages. These techniques are successful to a certain extent in providing the best results possible for their intended indications. Hair transplantation involves the extraction of follicular-grafts, which are usually taken from the back or side of the scalp. These grafts are then implanted at the recipient’s site. These two techniques use the same procedure, but the grafts are harvested differently.
- The FUE technique involves the extraction of hair follicles with their roots and some surrounding intact tissue. The follicular transplants are implanted in the recipient area, taking into consideration aesthetics. FUT Hair Transplant Technique (Strip Technique) involves the harvesting of a long strip of hair from the side and back of the head. This strip contains thousands of follicular units. - There is still debate and discussion about which technique is best, but in the end no technique is considered superior. The indications are based on the aspects that were discussed during the consultation. The number of hairs needed or the extent of baldness is what determines the most important factor. The FUT method is recommended for large baldness such as Norwood grade IV and V. This technique can provide up to 3000-3500 hair grafts. In initial balding cases or for facial hair transplantation, FUE is preferred because it can successfully recover 2000-2500 grafts.

How is the combination technique for hair transplant performed?
- Local anaesthesia administered:- Dr Sareen prepared a special cocktail of multiple anaesthetics that provides a long-lasting action. It is only necessary to administer the cocktail once throughout the entire procedure. The effects of the local anesthesia last for about 12 hours, which is longer than the time needed to perform hair restoration by any method.
- Graft harvesting:- Combining FUE and FUT hair transplant techniques is a technique for graft harvesting. The FUT technique is used to harvest the maximum number of hair follicle grafts, followed by the FUE technique for the remaining grafts.
- FUT or Strip Technique: An approximately 1-1.5 cm wide strip is taken from the back and sides of the head depending on the laxity and number of grafts needed. In cases of pattern baldness or extensive hair loss, approximately 2 cm is left under the strip. This area is saved for future sessions. - FUE (Follicular Unit Extraction):FUE is used after FUT to extract the remaining number of grafts. This method can extract 500-1000 grafts. The FUT technique harvests the maximum number of grafts. As a result, FUE is used to extract any remaining grafts. The technique has the added benefit of being able to extract hair follicles at a distance, as fewer grafts are required. It reduces the damage rate and helps to prevent overharvesting. This helps to avoid the violation of the safe donor area as well as the depletion of density at the donor site.
- Body Hair Transplant: The chosen harvesting area is called “shadow area”, which is located under the chin. It was not recommended earlier due to the possibility of scarring. In Sareen Hair Clinic, Dr Sareen prefers to use a 0.75mm punch to extract beard hair, which eliminates the chances of scarring. What are the indications for (Body Hair Transplantation) combination technique?
